Are you a new patient a Greenspan Wellness Center? To start, let us just say we are happy you chose us to help you with your condition at our beautiful wellness center in the heart of Black Mountain, North Carolina. At your first appointment, a member of our team will sit down with you, get to know you better, and put together a plan to help you overcome your challenges. We will also answer any questions you have about our approach at this time. If you have yet to make an appointment, please call our office so we can get you scheduled as soon as possible.
Patient Forms
Greenspan Wellness Center offers our patient intake form as a fillable pdf. Please complete it in the convenience of your own home or office, before you arrive at our office.
1. Please download the form.
2. Complete it by filling out every section.
3. Click the Save button at the top of page 1 to save the completed form on your computer (a box on your computer will come up asking where you want to save it).
4. After it’s saved in that location email it as an attachment to info@greenspanwellnesscenter.com
